The quotation form collects business contact details and programme-scoping information. It is not designed to collect employee health information. Technical security logs may include IP address, browser information and the page used to submit an enquiry.
How data is used
Enquiry data is used to respond, scope requested services, prevent abuse and maintain an appropriate business record. A lawful basis, controller identity, retention period and processor list must be confirmed before launch.
Sensitive information
Do not submit employee symptoms, diagnoses, medications, test results or other health information through the general form. If clinical services are commissioned, personal health information requires a separate secure process and appropriate privacy information.
